Government-Issued Photo ID

Florida law requires that every signer be identified through satisfactory evidence. Please ensure all signers have one of the following (must be current or issued within the past 5 years):

  • Florida Driver's License or ID card issued by the DHSMV.
  • U.S. Passport issued by the US Department of State.
  • Foreign Passport (must be stamped by the U.S. Citizenship and Immigration Services).
  • U.S. Military ID.

Note: The name on your ID must match the name on the document being signed.

  • Do Not Sign Yet: Please do not sign or date the documents until I am present.
  • All Pages Present: Ensure you have the entire document, not just the signature page. I cannot notarize a signature on an incomplete document.
  • Filled In: All blanks (except for signatures and dates) should be filled in before I arrive.

Required Witnesses

  • Check Your Document: Review your document to see if "Witnesses" are required in addition to a Notary (common for Deeds, Wills, and Powers of Attorney).
  • Who Can Witness: Witnesses must be at least 18 years old, disinterested in the transaction (not a beneficiary), and have their own valid photo ID.
  • Need Help? If you cannot provide witnesses, please let me know in advance so I can attempt to bring a professional witness for an additional fee.
